Promover la equidad y abordar las disparidades sanitarias entre hispanos, latinos y otras comunidades vulnerables a través de la investigación, la defensa y los servicios culturalmente adaptados.
HHC is a nationally recognized, statewide community-based organization based in Hartford, CT. Over multiple decades we have established a deep relationship of trust and respect with the communities we serve through our commitment to social justice and direct care. Annually we provide services to over 15,000 individuals throughout Connecticut. The great majority of these community members are people of color who in a lot of cases do not speak English and/or are refugees or immigrants. It is important to note that over 50% of Hartford, the state capitol of CT, is Latino or Afro-Latino.
We work towards meeting our mission through three core pillars:
1. We conduct community-based participatory research with federal organizations, and local universities such as Yale University and the University of Connecticut. Our work has been recognized by the World Health Organization and UNICEF.
2. We conduct system and policy advocacy for and with the community, to assure that structural conditions are in place for all people to achieve their highest level of health and amplify community voices and influence. To this end, we are committed to an advocacy agenda that is data and community driven.
3. We provide culturally competent evidence-based services. 100% of our services are based on 7 core tenants: 1) cultural competency (e.g. Over 90% of our qualified staff are Latino and almost 100% are bilingual); 2) learned experience; 3) integrated service and delivery models; 4) collaboration with the public, private and non-profit sectors; 5) sustainability; 6) best practices; and 7) equity focused.
Community Outreach – We provide community outreach services with community health workers and patient navigators for pregnant women, school-aged children, adults, seniors, the disabled, immigrants, migrants, refugees, and people who are HIV positive through our:

- Health Protection and Education Center provides culturally sensitive community-based services to protect the health of individuals and communities focusing on the prevention and mitigation of infectious, chronic diseases and violence. We also provide supportive employment and case management services to improve the well-being of clients with a history of addiction or mental health conditions.

- Community Nutrition Center offers programs and services designed to improve healthy lifestyles including nutrition and physical activities across the life course in socioeconomically vulnerable communities. Our nutrition services have been recognized by First Lady Michelle Obama at the White House.
- Youth Services provide a continuum of services to support and guide families collaboratively through childhood and adolescence. Programing includes academic support, cultural activities, youth leadership development, and parenting support. Some of the programs we offer include the Family Resource Center at Maria Sanchez School, the Extended Learning Program at Maria Sanchez School, and Bridge to Success.
Wellness Services – We provide wellness services at our Family Wellness Center for the general community. We adopted the family wellness wheel which includes 8 domains of wellness (social/emotional, physical, financial, environmental, spiritual, occupational, intellectual, environmental). We are collaborating and integrating our services with 15 different agencies. This network is referred to as “The Anchor Partners”. All of services are provided in our facility, in the heart of the Latino community, and coordinated by HHC.
Clinical Services – We provide a variety of integrated clinical services in collaboration with 7 different agencies:
- Integrated Behavioral Healthcare (Compassion Center) services at our Main Street and Park Street offices for people who have mental health and substance abuse challenges and who are victims of intimate partner violence. These services are physically integrated with an on-site FQHC (Charter Oak Health Center), and a hospital system (Hartford HealthCare).
- Migrant Health Clinic services at our Farmington clinic, employment sites (farms, plants), and the community for migrant workers. These services are conducted in collaboration with the University of CT School of Medicine.
- Pharmacy services at our Main Street office. These services are provided in collaboration with BOCA Pharmacy.
- Maternal and Child Health Center services at our Main Street office for women before, during, and after pregnancy (Doula, Comadrona, Healthy Start, Stronger Families Stronger Futures). Services are delivered by community health workers, at hospitals, homes, virtually, by telephone, or any other comfortable setting. These services are provided in collaboration with WIC, the Village, and Hartford Hospital’s Women’s Health Clinic.
Welcome Center – We provide a supportive entry point for immigrants and refugees. In collaboration with multiple partnering agencies (e.g. Greater Hartford Legal Aid, school district) we provide community orientation sessions, ESL classes, legal support, socialization/peer-to-peer opportunities, a resource lab, and access to community resources.
